Every procedure has a name against it
Not a department, a person. A procedure whose owner left is the most common thing in a neglected register, and it is invisible until somebody follows the document and finds it wrong.

One current version, and the ones before it
Two files called onboarding-final and onboarding-final-v2 is not version control, it is a coin toss. The register says which one is current and keeps the others readable rather than deleted.
